Accidental Death
Required Documents
- Letter from the beneficiary requesting compensation. The letter should have all the personal data, policy number and the reason for the claim.
- Copy of the Insured’s personal identity document
- Completed Loss Claim Form
- Copy of the airline ticket
- Insured’s birth certificate
- Death Certificate from the Public Registry (original and certified copy).
- Beneficiaries’ birth certificate
- Beneficiaries’ Personal identity card
- If the beneficiary is under age, a copy of the personal identity card of his/her legal representative.
- In case that there is no named beneficiary, original and certified copy of the Probate.
- Autopsy report
- Report from a competent authority that intervened in the accident.
- Any other document that the company considers necessary.
Loss of Luggage

- Completed Loss Claim Form
- Copy of the Insured’s personal identity document
- Airline Ticket copy.
- Copy of the luggage ticket from the transport company.
- Copy of the deft or loss report issued by the transport company.
- Any other document that the company considers necessary.
Medical Emergency Transport
Required Documents
- Completed Loss Claim Form
- Copy of the Insured’s personal identity document.
- Copy of the airline ticket
- Opinion from a doctor certifying to the injury’s seriousness or nature that justifies a medical emergency transport.
- Vouchers of expenses incurred.
- Any other document that the company considers necessary.
Repatriation of remains
Required Documents
- Completed Loss Claim Form
- Copy of the Insured’s personal identity document
- Copy of the airline ticket
- Original Death Certificate from the Civil Registry.
- Vouchers of expenses incurred.
- Personal identity document from the person that paid the expenses.
- Any other document that the company considers necessary.
Medical Emergency for Travel

- Copy of the Insured’s personal identity document
- Airline Ticket copy
- Doctor’s opinion certifying the illness.
- Copy of the page of emergency attention.
- Vouches of the incurred expenses
- Any other document that the company considers necessary.
Delayed Luggage
Required Documents
- Completed Loss Claim Form.
- Copy of the Insured’s personal identity document.
- Airline Ticket copy
- Copy of the luggage ticket from the transport company.
- Copy of the transport company’s report where it indicates the reason for the delay, flight number, date and time of the luggage’s arrival.
- Vouches of incurred expenses for personal articles due to the delayed luggage.
- Any other document that the company considers necessary.
Travel cancellation
Required Documents
- Completed Loss Claim Form
- Copy of the Insured’s personal identity document
- Voucher of purchase of the travel ticket.
- Copy of the document attesting the reason for the flight interruption or cancellation.
- Copy of the marriage or birth certificate that attests the kinship with the family member for which the trip was cancelled, if applicable.
- Vouches of travel expenses not used and not refundable.
- Any other document that the company considers necessary.
